Workshop 3: January 22, 2027 - Challenging Diagnoses in Pediatric Renal Path
This workshop will be divided into 2 portions. The first is a brief lecture on a pattern-based approach to biopsies. Because renal diseases have a pattern of injury that is not specific to a given diagnosis, this initial portion will focus on a brief review of patterns of injury by light microscopy and their implications. This will be a review for practicing pathologists and a base for trainees. Accordingly, we will also use this opportunity to discuss nuances of patterns that can be challenging for seasoned renal pathologists and can create diagnostic dilemmas such as differentiating florid mesangial hypercellularity from endocapillary hypercellularity and defining a membranoproliferative pattern of injury.
Following this we will present 2 cases with in-depth discussion of the differential diagnosis and a focus on a practical approach to sign out. Each case will be interactive with opportunities to ask questions throughout as well as a dedicated question time at the end. We are open to considering using scanned slides available prior to the presentation or de novo presentations. The first case will focus on infection-associated glomerulonephritis and C3 glomerulonephritis. Though clinicians often ask for a definitive differentiation, these entities exist on a continuum, and this is not always possible, especially in the acute presentation. We will discuss some features that may help differentiate the two and how to phrase diagnoses and comments and address clinician concerns. We have allotted more time for this as it is an issue that comes up frequently in our experience. The second case will discuss membranous-like glomerulopathy with masked IgG kappa deposits with a discussion of proliferative glomerulonephritis with monoclonal immunoglobulin deposits (PGNMID) and membranous GN including membranous subtypes. As more targeted therapies are available and pediatric hospitals increase the upper age range of patients they serve, these are becoming increasingly important to recognize. The discussion will include why identifying a monotypic process is important, when to do kappa and lambda, and cover ancillary testing that can help in difficult ICDD.
Because we would like this to be interactive and allow participants to ask questions and raise practice concerns, we have opted to discuss fewer cases. However, if there is additional time we will have additional slides prepared.

Target Audience
The education design of this activity addresses the needs of physicians and scientists in the field of pediatric, perinatal and general pathology who are involved in the diagnosis and/or treatment of pediatric patients with pathologic disorders.
Learning Objectives
1. Be familiar with patterns of glomerular injury, variants, and pitfalls
2. Understand the clinical and pathologic considerations when differentiating between C3GN and infection-associated GN
3. Understand the nuances of phrasing final diagnoses in C3GN and infection-associated GN
4. Be able to recognize monotypic diseases in the pediatric population and know when to employ additional diagnostic tools to help clarify monotypic processes and unusual immune complex deposit diseases (ICDD)
