Workshop 1: January 9, 2026 - Patterns of Colonic Mucosal Injury in Chronic Diarrhea: A Diagnostic Approach to Pediatric IBD and Mimics
This case-based review course will provide an approach to the diagnosis of pediatric IBD, its conventional subtypes, the evolving patterns of monogenic and very-early-onset IBD and a wide variety of IBD mimics. This course will highlight histologic features to assign the most appropriate diagnosis and provide further guidance to the gastroenterologists.
Cases to be presented for IBD-mimics include:
1. Chronic granulomatous disease
2. Adenovirus colitis in an immunocompetent patient
3. Chronic ischemia secondary to myointimal hyperplasia of the mesenteric veins
4. Colonic Crohn’s and UC.
5. First diagnosis of Crohn’s and bi/tri-crypt adenomas at colonoscopy
6. Checkpoint inhibitor-induced pan colitis
7. CMV gastroenterocolitis
8. Monogenic IBD – apoptotic phenotype
9. Monogenic IBD – intraepithelial lymphocyte phenotype
10. Autoimmune enteropathy

Target Audience
The education design of this activity addresses the needs of physicians and scientists in the field of pediatric, perinatal and general pathology who are involved in the diagnosis and/or treatment of pediatric patients with pathologic disorders.
Learning Objectives
1. Identify the histomorphologic patterns of colonic mucosal injury and characterize histologic clues to differentiate from typical IBD.
2. Recognize various causes of chronic diarrhea, that clinically and endoscopically mimic IBD.
3. Understand the impact of pathology diagnosis to facilitate appropriate workup and management of pediatric patients.
